The Healthy Heart Society of BC is a catalyst for change in healthcare in British Columbia. It provides knowledge and support for improved prevention and management of chronic conditions.

The Society is an independent non-profit organization that helps accelerate change through promotion and cultivation of evidence-based practice for improving patient care and a capacity-building approach that turns ideas into action.
 
A partnership between healthcare professionals, industry, communities and volunteers, the primary, but not exclusive, focus of our work is cardiovascular disease.
 
The mission of the Healthy Heart Society is to measurably decrease morbidity and mortality from cardiovascular disease in British Columbia.
 
The Society works with others to build the capacity of communities to implement evidence-based disease management and prevention programs.  

What we will accomplish

 
The Society will help improve the lives of British Columbians, the health of communities and the professional satisfaction of healthcare providers by supporting the application of best clinical practice province-wide to achieve: 
 
  • Activated communities
  • Improved patient self-management
  • Delivery system redesign
  • Improved decision support systems
  • Better clinical information systems
 
We will be a recognized leader and innovator in British Columbia, a partner and employer of choice, and the first place to turn for expertise, help and encouragement for those who want to change British Columbia’s healthcare system for the better. 

How we will know our work has impact

 
We will establish cross-sector partnerships with a broad coalition of key stakeholders within and outside the healthcare system to achieve improved population health outcomes, such as:
 
  • Reduction in key indicators such as age-standardized mortality rate, hospitalization rates, numbers of complex clinical procedures, rates of incidence of chronic conditions such as chronic heart failure, diabetes, kidney disease, etc – based on Health Authority and Ministry of Health Services administrative data
  • Increases in guideline care, reduced health costs, improved provider satisfaction and autonomous patient self-management as determined by independent surveys
