hearts@work is an award winning program designed to support people in improving their health. It focuses on the prevention of cardiovascular disease and other lifestyle related conditions through risk factor screening, education, motivation-based counselling and goal setting. 

A heart health literacy campaign

  • For health professionals, hearts@work provides resources and training for community-based health promotion events

  • For members of the public, a hearts@work event is an opportunity to check risk factors for cardiovascular disease and diabetes and set personal health improvement goals

Goals

  • Identify individuals at high risk for cardiovascular disease, diabetes and other chronic conditions
  • Assess the degree and nature of their risk
  • Help them set lifestyle goals based on a self-assessment approach
  • Encourage and assist them to access treatment and local resources
  • Follow-up to encourage and support positive lifestyle change
